Appointment and post-hire requirements
To process appointments of employees, employee need to prepare appointment papers and submit to the hiring department for processing at HRDO. Paperwork will go through a process. Once approved, the employee need to accomplish post-hire requirements. Additional information can be downloaded here.
NOTE: Failure to submit appointment papers and posthire requirments will delay the issuance of your salary.
1. Bring ID picture 2×2 to HRDO (Quezon Hall building) 2nd floor right side (mezzanine) to obtain and fill out the Application for Identification Card
Information needed in filing up Application for Identification Card (Wait for the staff to check and approve the Application form):
a. Employee number (found in appointment paper)
b. Blood type
c. GSIS number
e. Phone number of person to notify in case of emergency
2. Bring the approved form to OUR building (2nd floor left side):
Have your photo taken. Verify your information in the ID and affix your signature. Wait for the ID card to be printed.
University email address
Setup university email: UP System Email (@up.edu.ph) – Use this as your official work email address, CRS and UVLE accounts
1. Bring your UP ID and Appointment to UP-ITDC (Vidal Hall) and request for a up.edu.ph email account
2. Please ensure to put a Privacy and Confidentiality Notice in the email signature.
